The E-Sign On-Line™ process at UCLA is the convenient, easy way to complete your Perkins Loan document package, with 24/7 availability using almost any computer with internet access. The online Perkins Loan documents package consists of a(n):
- Personal Data Form
- Entrance Rights and Responsibilities Form
- Federal Perkins Loan Master Promissory Note
Signing the Federal Perkins Loan Master Promissory Note is similar to signing a Stafford Loan Master Promissory Note in that it can be done electronically. When completed, the promissory note remains active at UCLA for up to 10 years. In using this method, there will be no need to fill out another set of Perkins Loan documents when accepting future Perkins Loan offers on your Financial Aid Notification (FAN).
The online process begins here on the UCLA Student Loan Services & Collections website. This process also incorporates the services of Affiliated Computer Services (ACS) and its E-Sign On-Line™ website where students electronically sign the Federal Perkins Loan Master Promissory Note.
TO DETERMINE IF YOU SHOULD PROCEED with the E-Sign On-Line™ Process at UCLA, please check to see if your current situation matches any one of the following statements:
(PLEASE NOTE: A period of at least 2 days must pass after accepting the Perkins Loan offer before the electronic master promissory note will appear on the ACS website. Also, if you have previously signed a promissory note electronically through the E-Sign On-Line™ Process at UCLA, you will have access to viewing the e-signed document on the ACS site.)

- I accepted an offer of a Federal Perkins Loan on my most recent FAN AND I HAVE NOT electronically signed a Federal Perkins Loan Master Promissory Note while at UCLA within the past 10 years.
